The two participating CPC+ commercial payers (Priority Health and BCBSM) have engaged a team of colleagues from the University of Michigan to support multipayer CPC+ coordination in Michigan. Our goal is to optimize the success of CPC+ practices in the five year demonstration and to build for sustainability beyond 2021 (the ending year of the CPC+ federal demonstration). The services provided by the U of M team for CPC+ implementation include:
- Convening, Governance, Coordination, and Partnership Building which includes:
- Coordinating multipayer leadership
- Supporting CPC+ governance groups and subgroups
- Building relationships with key stakeholders to inform progress
- Communicating with key federal and vendor partners for synergy building on the foundations established for advanced primary care in Michigan
- Facilitating the removal of roadblocks that the CPC+ Michigan practice and PO community encounter with payers, federal partners and their vendors.
- Strategy formation and implementation for ongoing post-demonstration sustainability
- Multipayer Data, Reports and Dashboards
- Acquisition of claims, eligibility and other data from each payer
- Compilation of data and dashboard design
- Provisioning of data
- Production of reports and other resources

In Michigan, the governance model consists of:
- Payer Leadership Group – Representatives from each participating commercial CPC+ payer who advise on their approaches to CPC+ implementation and fund multipayer design and resources/support for the CPC+ practice and PO community.
- Steering Committee – Representatives from the practice, PO and system community who participate in CPC+.
- Subcommittees, including Care Interventions, Dashboard and Performance Oversight, Metric Alignment, and Payer Recruitment and Leadership
